Google is adding new health tracking to the Pixel Watch 3, 4 and 5 through a free update. It brings blood pressure trends, insulin resistance trends and sleep breathing reports. Owners of older models do not need to buy new hardware to get them.
What the health tools do
Blood pressure and insulin resistance checks show up once a month. They track how things change over weeks, so they are not instant readings. The article is clear that these are wellness indicators, not diagnostic tools. Sleep breathing works faster: a report each morning shows how your breathing varied overnight.
Google says the approach uses Health Foundation Models, which are large computer models trained on health data, instead of new sensors. That helps explain why older watches qualify. The insulin model was trained on more than a trillion data points from over five million users.
Safety feature and limits
Breathing Emergency Detection can start an emergency call when the watch sees severe, lasting drops in blood oxygen. On the Pixel Watch 4 and 5, it is limited for now to select European markets, including France, Germany, Ireland and the UK.
Cost is another catch. Fitbit Air owners can reach the new tools only with a Google Health Premium subscription. Pixel Watch owners get them for free.
Gemini and GPS on the Pixel Watch 5
The September 2026 update lets Gemini, Google's AI assistant, handle some tasks on the Pixel Watch 5 without an internet connection. The update installs automatically overnight or when the watch is on a charger. Proactive Suggestions need a Pixel Watch 5 paired with a Pixel 11 that has Gemini and proactive assistance turned on.
Smart replies now offer three choices instead of one, and Google says they are about 50 percent faster. Android Authority notes the September changelog mostly recaps launch features, so expect steady refinement rather than a big leap.
Google also claims the Pixel Watch 5 has twice the GPS accuracy of earlier models in difficult environments. It says it beat the Apple Watch Ultra 3 and Garmin Fenix 8 Pro in an internal July study.
What is still unproven
- The GPS claims come from Google's own study, and independent comparisons are still needed.
- Many users say the Pixel Watch 5 is slower than an Apple Watch at detecting cycling.
- Breathing Emergency Detection is not yet widely available outside select European markets.
The Apple Watch does not yet offer insulin resistance trends, according to the article. For Pixel Watch owners, the free health update is the clearest reason to stay with the platform.
The takeawayIf you own a Pixel Watch 3, 4 or 5, watch for the free update, and treat the new health trends as hints to discuss with a doctor, not as test results.
